• For those who fill out DYI Assessment Form, Dr. Kim will send you an e-mail of recommended membership level. 

• Individuals with more complex health needs may be recommended the diamond membership tier

• Individuals with simpler cases may be eligible for lower-tier memberships.

• Whenever it’s suitable, Dr. Kim will suggest when it might be appropriate to transition to a lower tier—however, you remain in control of these decisions at all times.
